Roberts Elementary Registration
REGISTRATION FOR NEW ZONED STUDENTS BEGINS IN APRIL AND CONTINUES INTO JUNE.
The following documents are required for all students enrolling at Roberts.
Original documents are required; we will make a photocopy and return the original.
_____ A current electric bill in the name of the parent with whom the student resides to confirm residence in the Roberts zone
_____ Current driver's license of the same parent showing the same address as the electric bill
_____ Birth Certificate - official state birth certificate required for students born in the United States; Passport requested for students born in other countries
_____ Immunization records including all of the following:
*DPT series - 5 doses, last booster after fourth birthday (4 doses if 4th given after 4th birthday)
*Polio series - 4 doses, last booster after fourth birthday (3 doses if 3rd given after 4th birthday)
*MMR - 2 doses given after first birthday
*Hepatitis B - 3 doses
*Hepatitis A – 2 doses required for students entering KN in 2009
*Varicella (Chicken Pox) 2 doses required for students entering KN in 2009
(or certification from parent that the child has had the disease)
*Students enrolling in Pre-K will also be required to have the appropriate doses of PCV7and Haemophilius Influenze Type B (Hib) according to the current Immunization Requirements for School Attendance schedule.
_____ Social Security card (optional - requested if student has SS#)
_____ In cases of divorced parents, the legal court decree showing custody of the child is required - the only pages to be copied are those showing the custody arrangements and the page which has a seal, date and judge's signature
_____ Students enrolling in First through Fifth grades need last report card or checkout sheet from previous school and address of previous school so that complete records can be requested.
